Director of Occupational Therapy ServicesJason Melnyk, OTR/L, MS, CEAS II has been a licensed Occupational Therapist since graduating with a Bachelor’s (2001) and Master’s degree (2002) in Occupational Therapy from Thomas Jefferson University. Jason is a Certified Ergonomics Assessment Specialist – Level II, having received these credentials from the Back School of Atlanta and is trained in administering Pre-Placement Post-Offer Assessments, Job Demand Analysis, and Functional Capacity Evaluations (FCEs).
His areas of clinical expertise are derived from multiple years in an academic healthcare center, acute care, and outpatient service delivery. Jason is licensed in both New Jersey and Pennsylvania and has been a contributing faculty member to the Thomas Jefferson University Occupational Therapy Department.
Currently, he serves as the Director of Industrial Rehab Medicine and Lead Functional Capacity Evaluator at STRIVE Physical Therapy & Sports Rehabilitation with over 30 locations within New Jersey and the greater Philadelphia area.

Occupational TherapistEric’s interest in hand and upper extremity rehabilitation was inspired by his grandfather’s recovery from a traumatic degloving injury. This experience sparked a lasting passion and dedication to this specialized field of Occupational Therapy.
Eric graduated from Thomas Jefferson University’s accelerated Occupational Therapy program, earning his Bachelor of Science in Occupational Health and Master of Science in Occupational Therapy. During his graduate studies, Eric completed a clinical internship at the prestigious Philadelphia Hand to Shoulder Center in Center City Philadelphia, where he developed foundational skills in upper extremity rehabilitation.
Eric achieved his Certified Hand Therapist (CHT) credential and completion of the renowned Virtual Hand to Shoulder Fellowship under the guidance of Mirella Deisher, OTD, CHT. The 48-week curriculum is centered around the text Rehabilitation of the Hand and Upper Extremity, studied by CHTs and orthopedic hand surgeons. The fellowship featured guest lectures from internationally recognized experts, including Dr. Don Lalonde, MD and Dr. Elisabet Hagert, MD, PhD. Eric also holds a certification in Functional Capacity Evaluations (FCE), allowing him to assess patients’ physical abilities to support safe return-to-work planning.
Outside of his clinical work, Eric enjoys staying active through swimming, hiking, and kayaking. In his leisure time, he also finds fulfillment in reading non-fiction, restoring classic cars, and attending car shows.
